Sand Toe

What is sand toe?

Sand toe injury is a sprain to the big toe joint commonly seen in volleyball where the big toe joint is hyper plantarflexed. This usually occurs when the big toe hits an unexpectedly firm divot in the sand or during a jump serve or a running spike.


There is usually pain, swelling, and bruising surrounding the big toe joint. You may or may not be able to bear weight.


There may be instability of the joint on exam. X-rays will be taken initially and bilateral films are helpful for comparison. Sometimes an MRI may be required.


Surgical treatment of sand toe is almost never required unless there are other associated injuries. Treatment consist of rest, ice, compression, elevation (RICE) and taping. In severe injuries a period of immobilization in a cast boot or cast may be required. Physical therapy is important in the rehabilitation phase of recovery.
